Goals and Objectives – To eradicate the problem or at least to reduce consequences of accidents involving people working in enclose spaces.
How – By organizing competition for the best idea how to deal with this issue. This will require you to brainstorm with you crew and fill up the excel sheet attached.
Audience – Mainly seafarers but other stake holders as well as they are very often part of the problem and not a solution as yet.
Best solution could be simple, user friendly procedure, change or technology gadget which would be universally used by our seafarers.

How to reduce over 90% consequences of accidents involving people
working in enclose spaces
I had been working as C/Offfor more than 10 years and I didn’t have any accident or incident in the enclose space because I always thought 5 moves ahead before doing something. Each member of the crew knewthat without my permission nobody couldenter to the enclose space. And if I sent people there I knew that it was perfectly safe because I gave all necessary instructions how to prepare and checked the atmosphere, all operations were carried out under my supervision. Also I didn’t haveany pollution during cargo operation because I also thought 5 moves ahead to exclude any human mistakes.
We have very good procedures but we still lose our people in enclose space. We also know that increasing the procedures, arrangements, posters and crew training for entering to enclose space won’t give us desire effect because there is following depending:
So, I consider there is only one way to solve this problem.
1) It’s necessary to make C/O as a person in charge think as minimum 3 moves ahead.
2) I think everybody knows that it’s in human nature that he starts both thinking and working perfectly in case it touches his personal interest.

I offer the following idea:
C/Officer as a safety Officer should be charged fees every month (for example 50$ or 100$ ) if there are not any incidents or accidents involving people working in enclose space (we also may include here working aloft).But he can get this money only when he leaves the company without incident or accident(or promoted to Master)..We can name it for example “leave company without incident or accident”. If there is any accident or incident during his work he will lose all his money. The more money he will have on his “account” the better he will think about safety.For example, when he will have 5000 $ on his “account” I am sure he will do his best not to lose his money.Everything will be checked and rechecked and everyone will be properly instructed and all operations will carry out under his supervision. I believe only this practice can eradicate the problem.
I am sure If all companies put it into practice they will save many lives and a lot of money.
